The form captures the employee's name, department, job title, and manager alongside the performance period start and end dates. Capturing the manager's name within the request record ensures the approval is directed to the correct person and that the accountability chain is documented from the point of submission.
Target and achieved sales
The form captures the employee's target sales amount and their achieved sales amount for the period. These two inputs are the foundation of the entire bonus calculation — everything that follows is derived from them automatically by the form, without requiring the employee or manager to perform any calculation or tier lookup.
Automatic achievement calculation and tier classification
Once the target and achieved amounts are entered, the form calculates the achievement percentage automatically. Based on this percentage, the form determines the applicable bonus tier — for example, Tier 2 for achievement between 90% and 99% of target — and displays the tier classification within the form. The form then applies the corresponding bonus percentage for that tier and computes the final calculated bonus amount. Every calculation is consistent, rule-based, and transparent, eliminating the manual arithmetic errors and inconsistent tier classifications that occur when bonus calculations are handled in spreadsheets or communicated informally.
Reason, supporting documentation, and manager approval
