How it works

Patient details

The form captures the patient's last name, first name, gender, and date of birth. These four fields establish the patient's core identity record and provide the demographic context relevant to clinical intake, ensuring that every appointment request is clearly attributed and consistently formatted from the moment of submission.

Contact and address information

A dedicated contact section captures the patient's street address, city, state, postal code or ZIP, and phone number, with an optional email address field. Consistent address and contact capture across every request ensures that the practice has the information needed to reach the patient, confirm the appointment, or send follow-up communications without additional follow-up to gather details that should have been collected at intake.

Appointment date

The form captures the patient's requested appointment date in a structured date field, giving the scheduling team a clear, filterable record of when each patient wants to be seen. The appointment list can be sorted and filtered by date, making it straightforward to manage the schedule and identify availability without maintaining a separate calendar or spreadsheet.

First visit flag
