Organizations that handle information requests through a general contact email address create an immediate triage problem. Requests arrive without a consistent structure, missing the details needed to route them to the right person or respond efficiently. There is no central record of what was requested, by whom, or whether it was followed up. And for teams managing high volumes of enquiries, the lack of structure means time is spent gathering basic information that should have been captured at submission.

The Information Request template for SharePoint, built with Infowise Ultimate Forms, provides a simple, structured form that can be placed on any internal or external page — giving visitors a clean way to submit a general information request with enough contact detail to allow the appropriate representative to respond by email or phone.

How it works

The request form

Visitors complete a structured form providing their contact details and the nature of their information request. The form captures the information needed to route and respond to each enquiry — name, email address, phone number, and a description of what they are looking for. The structured format ensures every request arrives with the same basic information, eliminating the need for an initial follow-up just to gather contact details.

Internal and external accessibility

Using the Ultimate Forms External Form feature, the Information Request form can be placed on a public-facing website or an external landing page, making it accessible to visitors without a SharePoint account. The same form can equally be deployed on an internal SharePoint intranet page for staff-facing information requests — for example, HR queries, IT enquiries, or departmental information requests — without any modification to the template.

Centralized request list

Every submitted request is stored as a structured record in a standard SharePoint list. The team responsible for handling enquiries has a single, searchable view of all incoming requests — filterable by submission date, contact name, or any other column — giving clear visibility of what has been received, what has been responded to, and what is still outstanding.

Extendable to match your process

The template is a ready-to-use starting point that can be adapted to any information request workflow. Additional columns can be added to capture the type of request, the product or service it relates to, or the department it should be routed to. Automated acknowledgement emails can be sent to the requester on submission. Assignment rules can route each request to the correct team member based on its category — all using Ultimate Forms Actions and Alerts without any development work.
