How it works
Issue submission — issues can be submitted by internal users directly through SharePoint, or by external parties through the Ultimate Forms External Form feature. This means contractors, customers, or other external stakeholders can report issues without needing a SharePoint account or access to your internal environment. Every submission follows the same structured format, ensuring all the information needed to act on the issue is captured from the start.
Assignment and ownership — once an issue is submitted, a responsible party is assigned to own it. The assignment is recorded in the issue record and triggers a notification to the assignee. From that point, ownership is clear and visible to everyone with access to the list — there is no ambiguity about who is responsible for what.
Task creation and assignment — resolving an issue often involves more than one action by more than one person. The template supports the creation and assignment of tasks linked directly to each issue. Each task has its own owner, and progress on individual tasks is tracked alongside the parent issue, giving managers a complete view of what is being done and what is still outstanding.
Resolution documentation — as work progresses, resolution steps are documented directly within the issue record. The full history of what was done, by whom, and when is captured in SharePoint — creating a complete, auditable record that is available to anyone who needs to review it, without having to piece together an email thread or ask the person who handled it.

The Issue Tracker system is built entirely on standard SharePoint lists. There is no external data storage and no custom interface. All issue records, tasks, and resolution documentation stay inside your SharePoint environment, governed by your existing permissions and data policies. The system can be extended or adapted at any time in the browser by the administrator who manages the site.
