Organizations and causes that collect donations through email, paper forms, or informal channels have no reliable way to track who donated, how much, or how to follow up with donors who expressed interest in giving on a recurring basis. Without a central record, donor relationships are difficult to manage and fundraising progress is hard to measure.

The Donations template for SharePoint, built with Infowise Ultimate Forms, provides a clean, accessible donation form that supporters can complete externally — without a SharePoint account — capturing their details and donation preference, with every submission stored as a structured record in SharePoint. The form supports both one-time donations and requests for contact to arrange monthly giving, and can be extended with payment integration for organizations that collect funds directly through the form.

How it works

The donation form

Supporters complete a straightforward form providing their contact information and indicating the nature of their contribution — whether a one-time donation or a request to be contacted to arrange a recurring monthly donation. The form is designed to be simple and accessible, removing any friction that might discourage a supporter from completing their submission.

Anonymous submissions

The form fully supports anonymous submissions, allowing donors who prefer not to share their identity to contribute without providing personal details. This flexibility makes the form suitable for a wider range of fundraising scenarios, including sensitive causes where donor confidentiality is important.

External accessibility

Using the Ultimate Forms External Form feature, the donation form can be embedded on any public-facing website or shared as a direct link — accessible to supporters anywhere without a SharePoint account. Every submission lands directly in a centralized SharePoint list, giving the fundraising or development team an immediate, structured view of all incoming donations and contact requests.

Payment integration

For organizations that want to collect payment directly through the form, the template can be extended with payment integration — allowing donors to complete their contribution within the same form experience. Refer to the Infowise documentation for guidance on adding payment capability to your donation form.

A centralized donor record

Every completed submission is stored as a structured record in a standard SharePoint list. The team managing fundraising has a searchable, filterable view of all donor submissions — by donation type, date, or any other column — enabling effective donor follow-up and accurate fundraising reporting without manual data consolidation.

Built on standard SharePoint lists

The system can be extended or adapted at any time in the browser by the administrator who manages the site.
