Product feedback collected informally through email replies, verbal comments, or unstructured suggestion boxes gives product and sales teams data that cannot be compared, aggregated, or acted on systematically. Ratings vary because no consistent scale was applied. Suggestions are scattered across inboxes. Without a centralized record linked to a specific product and purchase, identifying trends across customers or stores requires manual effort that should not be necessary.
The Product Suggestions template for SharePoint, built with Infowise Ultimate Forms, provides a structured feedback form that captures customer contact details, purchase context, five standardized satisfaction ratings, and open-ended suggestions. Every response is stored as a consistent, comparable record in SharePoint.
How it works
Customer and purchase details
The form captures the customer's name, email address, phone number, the product being reviewed, the purchase date, and the name of the store where the purchase was made. Capturing this context alongside the feedback ensures that every response is clearly attributed and that product and store teams can filter and segment responses by product, location, or time period without additional cross-referencing.
Five-point satisfaction ratings
The form presents five structured rating questions, each scored on a scale of one to five. The ratings cover overall product satisfaction, packaging suitability, clarity of instructions, product performance, and product affordability. Using a consistent scale across all five dimensions makes it straightforward to aggregate scores, identify which aspects of the product are performing well, and pinpoint where customer expectations are not being met.
Open-ended suggestions
A dedicated suggestions field invites the customer to provide qualitative feedback on the product, its performance, and the sales and service experience. This free-form field captures the specific, actionable insight that structured ratings alone cannot provide, giving product and service teams direct customer language to work with when prioritizing improvements.
