How it works

Visitor details

The form captures the visitor's name and the start date of their visit. Linking each feedback entry to a specific visit date allows organizations to track satisfaction over time, correlate feedback with specific events or periods, and identify whether service quality is improving or declining across different timeframes.

Five-dimension visit rating

A dedicated rating section asks visitors to score five dimensions of their experience on a one-to-five scale: overall visit quality, service satisfaction, cleanliness and maintenance of the facilities, likelihood to recommend, and how well the visit met their expectations. Using a consistent scale across all five dimensions makes it straightforward to aggregate scores, compare results across time periods, and identify which specific aspects of the visitor experience require attention.

Open-ended comments

Two open-ended fields give visitors the space to describe their experience in their own words. The first invites them to share details of what they enjoyed about their visit. The second asks specifically how the experience could have been improved.
