Training feedback collected on paper or through informal email follow-ups produces data that cannot be aggregated, compared, or acted on systematically. When questions vary between sessions, responses cannot be measured against each other. When results are stored in files or inboxes, identifying what is working and what needs to improve requires someone to manually collate information that should already be in one place.

The Training Feedback template for SharePoint, built with Infowise Ultimate Forms, provides a structured post-training feedback form combining rating scale questions with open-ended suggestion fields — with every submission stored in SharePoint as a consistent, searchable record.

How it works

The feedback form

Participants complete a structured feedback form following a training session. The form is built around a set of rating scale questions covering the key dimensions of the training experience — content quality, delivery, relevance to role, and overall satisfaction. Open-ended suggestion fields give participants the opportunity to provide specific comments or recommendations that a rating scale alone cannot capture. The combination of closed and open questions produces both quantifiable scores and qualitative insight in a single submission.

External accessibility

Using the Ultimate Forms External Form feature, the feedback form can be made accessible to participants outside your organization — for example, external delegates, contractors, or partners who attended the session — without requiring a SharePoint account. Fully anonymous submissions are supported for scenarios where participant confidentiality is important to the quality of the feedback received.

Centralized, comparable feedback records

Every completed feedback form is stored as a structured record in a standard SharePoint list. Because every submission follows the same format, responses are immediately comparable across sessions, trainers, topics, and time periods. Identifying sessions with consistently lower scores, tracking improvement over time, or comparing feedback by department or participant group is a matter of applying a filter rather than manually reviewing individual forms.

Customizable to your training process

The default questions cover the most common feedback dimensions, but the form is fully configurable. Questions can be rephrased to match your specific training context, rating scales can be adjusted, and additional columns can be added to capture information specific to your program — session name, trainer, location, or participant role — without any development work.
