Exit interviews conducted informally or not at all leave organizations without the data they need to understand why people leave. When feedback is collected through unstructured conversations or ad hoc email exchanges, it cannot be compared across departures, aggregated into trends, or used to drive meaningful change. The insight exists — it leaves with the employee.

The Employee Exit Survey template for SharePoint, built with Infowise Ultimate Forms, gives HR a structured, consistent way to collect feedback from every departing employee — using a combination of rating scale questions and free-form responses — and stores every submission in SharePoint as a searchable, comparable record.

How it works

The survey form

Departing employees complete a structured survey form built around rating scale questions covering the key dimensions of their employment experience — role satisfaction, management, workplace culture, career development, and any other areas your organization wants to measure. Alongside the rating questions, free-form feedback columns give employees the opportunity to provide context or raise specific points that a rating scale alone cannot capture.

Consistent, comparable data

Because every departing employee completes the same structured form, the responses are consistent and comparable across submissions. HR can identify patterns — departments with consistently lower scores, recurring themes in free-form feedback, changes in sentiment over time — that would be invisible in unstructured exit interview notes.

Stored and searchable in SharePoint

Every completed survey is stored as a record in a standard SharePoint list. Responses can be filtered by department, role, departure date, or any other column, making it straightforward to analyze feedback by segment without exporting data to a separate tool.

Customizable to your requirements

The template is a ready-to-use starting point. Questions can be rephrased or reordered, additional columns can be added to capture information specific to your organization, and a workflow can be implemented to route completed surveys to HR management or trigger follow-on actions — all without any development work.

Built on standard SharePoint lists

The Employee Exit Survey system is built entirely on standard SharePoint lists. There is no external data storage and no custom interface. All survey responses stay inside your SharePoint environment, governed by your existing permissions and data policies. The system can be extended or adapted at any time in the browser by the administrator who manages the site.
