NOTE: Starting with version 1.4.0.0 for SharePoint Online, the main app Ultimate Forms is being retired. Ultimate Forms Extensions app has been renamed Ultimate Forms and will now be the only app to be installed.

Ultimate Forms Extensions is a companion app of Ultimate Forms. It extends support of Ultimate Forms for Modern Experience user interface. Without this app, Ultimate Forms can only work in the Classic experience. It is not a stand-alone app and must be used in conjunction with Ultimate Forms.

Ultimate Forms Extensions adds the following capabilities:

• Modern forms - use Form Designer to create modern, responsive forms. Note that you are able to design Modern forms in Form Designer even when Ultimate Forms Extensions is not installed, but the forms will not load and will display an error message.
• Custom field support - allows custom field types of Ultimate Forms to render in modern list views.
• UI integration - adds buttons to the toolbar in Modern list views (Alerts, Actions, Print and Design).
• Client-side web parts (currently Microsoft 365 only) - provides alternative versions to the older (and soon to be deprecated) app parts of Ultimate Forms, such as Charts, Calendar and List Search. Additionally, comes with two brand new web parts, Filter and Rollup.

NOTE: it is recommended to always use our Wizard for installation. It will ensure all components are properly installed and deployed. Manual steps described below are not normally required.

If not using the wizard, Ultimate Forms Extensions for Microsoft 365 are available for download together with Ultimate Forms as one zipped package. Make sure to unzip and then upload, one by one, into your Enterprise Catalog.

On premises, the app will be added to your existing sites and to the catalog automatically. Only customers using SharePoint 2019 will be able to benefit from the app.

In Microsoft 365 starting with version 1.1.6.0, you can deploy Ultimate Forms Extensions (but not Ultimate Forms) centrally from your Enterprise Catalog, without the need to add it manually to each individual site. We recommend using this option for your installation, unless you specifically want precise control over where the app is installed. You can choose to deploy or not when you upload the app file (.sppkg) and agree to trust it:

If you already have previous versions of Ultimate Forms Extensions install within your sites and now you are planning to switch to centralized deployment, you will need to remove the app from the sites, otherwise the toolbar button will be duplicated (no actual harm will be done).
