How it works
Employee and incentive details
The form captures the employee name, employee ID, department, manager's name, email address, and optionally phone number alongside the incentive type and the incentive period start and end dates. Capturing the incentive type and period in structured columns ensures every claim is clearly attributed to the correct scheme and reporting period, enabling the finance and HR teams to reconcile claims against the applicable incentive plan without ambiguity.
Performance metrics
The form captures six performance metric columns: whether the sales target was achieved, the total sales amount, the bonus amount claimed, the number of new clients acquired, the number of contracts closed, the revenue generated, and the percentage of target achieved. Capturing all metrics in structured columns within the same claim record gives the approving manager and finance team a complete, verifiable performance picture alongside the bonus claim, without requiring separate reports to be cross-referenced before a decision can be made.
Other achievements and supporting documentation
An other achievements column gives the employee the space to document any additional performance or contributions not captured by the structured metrics. Two file upload columns allow the employee to attach sales reports and client feedback directly to the claim record. An additional documents column captures any other supporting materials. Storing all evidence linked to the claim in a single SharePoint record ensures the approving manager has everything needed to make a fully informed decision without requesting additional files.

The Sales Incentive Claim system is built entirely on standard SharePoint lists. There is no external data storage and no custom interface. All claim records stay inside your SharePoint environment, governed by your existing permissions and data policies.
