Deadline extension requests communicated through email or informal conversations leave managers without a consistent record of why the extension is being requested, what the original deadline was, and whether the proposed new deadline is reasonable given the circumstances. Justification for the extension is provided verbally or not at all. Without a centralized, formal request process, managing extension requests consistently, maintaining accountability for delivery, and reviewing patterns of recurring extensions across the team requires more effort than it should.

The Task Time Extension Request template for SharePoint, built with Infowise Ultimate Forms, provides a formal extension request form that captures the employee's details, the task or project title and description, the original deadline, the reasons for the extension, the proposed new deadline, a detailed justification, additional comments, and the supervisor or manager's approval decision. Every request is stored as a complete, auditable record in SharePoint.

How it works

Employee and contact details

The form captures the employee's full name, employee ID, department, position or title, email address, phone number, and supervisor or manager. Capturing the supervisor within the request record ensures the approval is routed to the correct person and that the reviewing manager has the organizational context needed to assess the request without additional follow-up.

Task identification and description

The form captures the task or project title and a full task description. Capturing the task description within the extension request gives the reviewing manager the context needed to assess whether the extension is reasonable relative to the scope and complexity of the work, without consulting a separate project plan or task log.

Original deadline and extension request

The original deadline is captured in a structured date column, creating a clear baseline for the extension request. The reasons for the extension are captured in a required column, and the proposed new deadline is captured in a structured date column. Recording both the original and proposed dates within the same request record makes it straightforward to assess the length of the extension being sought and to track whether the pattern of extensions on a given task or project suggests a systemic planning issue.

Justification and additional comments

A justification column gives the employee the space to explain in detail why the extension is necessary and what steps are being taken to complete the work within the proposed new timeframe. An additional comments column captures any further context the employee wants to communicate. Requiring a written justification at the point of submission ensures that the manager is reviewing a documented case rather than making a decision based on a verbal summary.

Supervisor approval

A supervisor or manager approval column captures the decision on the extension request. Documenting the approval within the same SharePoint record as the request details creates a complete, auditable extension record — from submission through authorization — without requiring a separate email confirmation or meeting note.
