Personal information

Residential address

A dedicated address section captures the applicant's full residential address — street address, city, state or province, postal or zip code, and country — in a structured format. Consistent address capture across all applications ensures that address data is reliable and usable without manual reformatting or clarification.

Education and identification details

The form captures the applicant's education level alongside their identification information — identification type, identification number, issuing country, and expiry date. An ID card upload field allows the applicant to attach a scanned or photographed copy of their identification document directly within the form. The uploaded ID is stored linked to the application record in SharePoint, eliminating the need to receive and match documents separately.

Account preferences

A dedicated account information section captures the account type and account category the applicant is applying for, alongside their monthly salary — providing the financial context relevant to account eligibility assessment. An additional services selection captures whether the applicant requires online banking, a debit or credit card, or a checkbook, enabling the processing team to prepare all required services alongside the account opening.

Accuracy confirmation

The form closes with a confirmation checkbox requiring the applicant to confirm that all information provided is accurate and complete. The Account Opening system is built entirely on standard SharePoint lists. There is no external data storage and no custom interface.
