Overtime requests managed through verbal agreements or informal email exchanges leave organizations without a reliable record of what was authorized, by whom, and why. Requests arrive without sufficient justification. Supervisors approve hours without a documented decision. Payroll processes overtime that was never formally authorized. And when labor costs need to be reviewed or an employee disputes an overtime record, the paper trail is incomplete.
The Overtime Request template for SharePoint, built with Infowise Ultimate Forms, gives employees a structured way to submit overtime requests — capturing the employee details, department, supervisor, hours requested, date of overtime, and justification — with a dedicated supervisor section that appears automatically when the supervisor accesses the form, allowing them to record their approval decision, add comments, and sign electronically within the same record.
How it works
The overtime request form
Employees complete a structured form capturing all the information needed to evaluate and authorize the request — the request date, the employee's name and department, the assigned supervisor, the number of overtime hours requested, the date on which the overtime will be worked, and the reason for the request. A detailed description field gives the employee the space to provide additional context beyond the stated reason — the specific work involved, the deadline driving the need, or any other detail relevant to the supervisor's assessment.
The supervisor approval section
When the supervisor accesses the form, a dedicated approval section becomes visible that is not shown to the requesting employee. This section captures the supervisor's approval decision, any comments they wish to add regarding the request, and their electronic signature confirming their authorization. The conditional visibility ensures that the approval section is presented only to the appropriate role — keeping the employee's view clean while giving the supervisor everything they need to document their decision within the same record.
A centralized overtime record
Every submitted request and its associated approval decision are stored as a structured record in a standard SharePoint list. HR and management teams have a single, searchable view of all overtime requests — filterable by employee, department, date, or approval status — giving full visibility of authorized overtime without relying on email threads or verbal confirmation.
