This control registers an alert directly for the item from on the form. By using this on your forms your users can, add an item level alert specifically for the record item that they are creating or editing. This is similar to the Add Alert icon on the list view page (item command bar).

 

This will create an alert to run based on the default configuration you provide for this control, the control config can be set to use the current users information to define the alert. Note the Alert for the item is only able to run when a record is edited, deleted or runs according to a date column. 

 

 

An alert will be created for the current item owned by the current user specifically for the item in the list after the form is saved.
