Instructions

Click New List

Select Blank list

Name your list “Support Requests” then click Create

On the command bar, click the Design link

Click the Form designer link

Click No in the dialog box.
(Do not generate default form.)

Drag the Container control onto the canvas.
(Left click, drag then release in white area.)

Drag the Button bar control directly below the container.
Notice the horizontal blue line indicating the drop location.

Your canvas should now look exactly like below.
Notice that the control that is selected has a blue border.
